1. Fill out the Summary of Benefits Request Form

This will not affect your benefits. A summary will be emailed to you within 2 business days.

2. We'll take care of the rest!

Once you fill out the Summary of Benefits Request Form, we will:

• Contact you with a Summary of your Benefits
• Set up a free optional eye exam with a trusted UVA partner (depending on your needs
• Set up an appointment with a UVA Sponsor to help you get the glasses you deserve!
